Your First Visit
We pride ourselves in providing efficient, yet highly effective chiropractic care. We understand that your time is valuable. Prior to your first appointment, please take a moment and complete the new patient forms. These forms can be emailed to info@shakerwomenswellness.com or printed for your convenience. This will allow Dr. Angela to begin your visit as quickly as possible.
At your first visit, please bring your new patient forms and insurance card. Dr. Angela will review the completed forms with you, perform a physical examination, order testing, and discuss your treatment plan with you. In most cases, your first treatment will also be given at this visit.
